The Importance of Data – Data Quality

Just like our peers, ADSTV is spending a lot of time these days looking for ways to improve data quality. The measurement of outcomes is such an important part of our quality improvement dialogue, yet this is an area that most community-based agencies are unable to provide in a sustained way. Pilot projects allow us to participate for brief periods of time however, what we need is a sustained focus on data quality and the measurement of impact of programming. Read More

Exciting News for Our Community Opioid Addiction Program (COAP)

Opioids are a class of powerful drugs that are primarily prescribed to treat severe pain. If opioids are abused, they can create feelings of intense pleasure or euphoria, and can also lead to fatal overdose, along with other medical, legal and social problems. Opioids include illicit drugs, such as heroin, as well as prescription medications, such as Percocet, morphine and codeine.

In May 2013, ADSTV was funded to provide a new program called the Community Opioid Addiction Program (COAP). COAP has 5 full time Addiction/Mental Health Counsellors. The purpose of COAP is to improve the health, social outcomes and quality of life of people experiencing Opioid Substance use—past or present. This includes Pregnant and Parenting Women, Youth and people living with HIV/AIDS/Hep C. The program also has a strong emphasis on providing support to people who are currently engaged in Substitution Therapy such as Methadone Maintenance Treatment, Suboxone, Buprenorphine or are considering using them as part of their treatment process. Each member of the team has a target of reaching approximately 200 unique individuals through individual, group counselling and educational sessions each year. Read More

New Program – Addiction Transitional Case Management Services

Addiction Transitional Case Management Service is a program offered through Addiction Services of Thames Valley.

The program consists of 3 Addiction Transition Case Managers (ATCM) with the overall goal of facilitating a seamless discharge process by ensuring that an individual has linkages to appropriate addiction and addiction-related services and supports upon discharge from the hospital. Together, the client and the ATCM will work together to create and implement a plan of care for enhancing stability and meeting the client’s most urgent addiction-related needs in order to reduce repeat visits to the Emergency Department and reduce the likelihood of readmission. London CAReS 3.0 – Now 24:7

Homelessness is a solvable problem through housing with support.

What makes London CAReS a success? London CAReS is a Housing First intervention for individuals experiencing chronic and persistent homelessness based on a highly collaborative community-based approach. As a best practice response to ending homelessness, the Housing First approach offers support in identifying and obtaining private market, scattered-site independent housing for individuals experiencing persistent and chronic homelessness, then provides the necessary intensive support (10:1) from a Housing Stability Worker to work towards community integration following an individualized support plan that is participant-driven and focuses on the participant’s personal goals and overall plan for wellness. Our Housing Stability support is not time-limited.

Street Outreach Services: London CAReS Street Outreach services operates 24 hours a day, 7 days a week; following the Five Pillars approach of Harm Reduction, Prevention, Treatment, Justice and Enforcement, and Community Collaboration and Integration. Read More
