The South West Local Health Integration Network (LHIN) is seeking members of the public to participate on its committees and offer their perspective on health care as a patient, family or caregiver and inform our decision-making around health system planning, funding and integration.

What are the benefits of taking part?

  •   Share your experiences and ideas about parts of the health care system that could impact your own experience and outcomes.
  •   Work with and learn from peers from across the South West LHIN.
  •   Build your listening and speaking skills as you share your own experiences and ideas.
  •   Learn a great deal about the health care system and the different initiatives designed to improve care.

The South West LHIN wants to hear from anyone who has been a patient, a family member of a patient, or a caregiver. This is a volunteer role. If you live within the South West LHIN’s geography and you would like to participate, visit to learn more or call 1-866-294- 5446 to speak with Pauline Adams. Deadline to apply is Monday May 1, 2017.
